Evidence of identity

To apply for documents which contain your personal information, you must provide evidence of your identity (ID), either when you apply or shortly after. If you are applying for documents for someone else and the documents contain their personal information, you need to show you are allowed to do this by providing evidence of your authorisation to act on that person’s behalf. You will also need to provide evidence of the person’s ID.

Examples of documents that may provide sufficient evidence of identity include:

  • a certified copy of your birth certificate, current driver licence, current passport, blue card or other photo identification card
  • A PDF extracted from the Queensland Digital Licence app of your driver licence, photo identification card, personal watercraft licence or recreational marine drivers licence. This must be emailed to the agency. You cannot post it.
  • copy of a prisoner’s identity card certified by a corrective services officer
  • statutory declaration of an individual who has known the applicant for at least one year. (A statutory declaration form can be downloaded at https://www.publications.qld.gov.au/dataset/statutory-declaration-affidavit)

Certified copies must be certified by a Justice of the Peace, lawyer, Commissioner for Declarations or Notary Public as a true and correct copy.

You may also be able to attend the FRC and show us your ID. Prior arrangements must be made with the Manager, Compliance & Legal Policy.

You cannot black out or remove any information from your ID before you give it to the agency. If you do, your ID will not be valid for the RTI Act. The FRC will ask you to send a new copy of your ID showing all the information. We will not be able to process your application until we have this.
